Course Description:

Urgent care centers are often considered the front door to the community's healthcare system, as a number of patients who arrive in an urgent care center will need to receive further care. In this session, attendees learn how to achieve safe and effective care coordination. The presenter reviews key processes for managing patient referrals, promoting safe patient hand-offs, and optimizing transitions as well as strategies for collaborating with other healthcare providers.

Learning Objectives: 

    •Identify three reasons why collaboration with other healthcare providers is vital to the success of the urgent care center (UCC).
    •Describe three ways the UCC can promote collaboration with other healthcare providers.
    •Discuss the impact of care coordination on patient outcomes, safety and satisfaction.
    •Identify critical elements for successful care coordination.
    •Design processes for effectively managing patient referrals.
